Reporting, approval flow, and comment bans

Added

Reporting. Readers can flag a comment with a reason. Reports land in a Filament resource with pending, reviewed and dismissed states.
Approval flow. Set require_approval and new comments are held until a moderator approves them. Authors still see their own pending comment.
Comment bans. HasCommentBan on your User model plus a comment_banned_until timestamp.
